Horizon BCBS New Jersey, JFK Medical Center partner for ACO

Horizon Blue Cross Blue Shield of New Jersey (Horizon BCBSNJ) and JFK Medical Center have created an accountable care organization (ACO) to improve healthcare outcomes for Horizon members. The ACO, part of JFK ACO, will launch on April 1.

Horizon will provide care coordination payments for the ACO to work toward increasing patient experience quality and control Horizon patient costs while improving population health.

“This collaborative agreement with JFK is another important step in transforming the health care system in New Jersey with an eye toward delivering better patient care while containing costs,” said Jim Albano, Vice President of Network Management and Horizon Healthcare Innovations at Horizon BCBSNJ.

The insurer offers other healthcare programs, including episode of care-based programs and patient-centered medical homes, serving over 500,000 of its members.
