MI Health Systems Implement Epic Healthy Planet Constellation

Six leading Michigan health systems target improved care cost and quality with Epic implementation.

- Affirmant Health Partners signed a deal with Epic to implement its population health management and accountable care platform — Healthy Planet Constellation — designed for clinically integrated networks.

The coalition of health systems is the first super clinically integrated network to adopt the platform.

The population health management platform will be used along with the existing Epic EHR and population health management software at Affirmant Health-affiliated hospitals and physician practices. Constellation is designed to collect and analyze clinical and claims data for actionable, timely patient data at the point-of-care.

This data will assist physicians in better-informed clinical decision making for improved patient health outcomes and reduced hospital spending.

“Our vision is to achieve a healthy Michigan at an affordable cost,” said President and CEO of Affirmant Health Partners William J. Mayer, MD. “Realizing this vision will require greatly enhanced information to support better care decisions by physicians and patients.  Epic’s Constellation will analyze data on our patients’ experiences across the care continuum to provide meaningful insights for physician and patient decision-making in real-time.”

Affirmant Health Partners is a physician-led network established in July 2015. The clinically integrated network comprises six leading health systems including Bronson HealthCare, Covenant HealthCare, Henry Ford Health System, Lakeland Health, MidMichigan Health, and Sparrow Health System. Affirmant Health is one of the largest networks in Michigan.

“Leveraging Constellation, Affirmant can identify variations in care and cost, standardize programs across chapters, and integrate insights directly into each organization’s Epic workflow,” said Epic Vice President of Population Health and Connect Alan Hutchison.

“These tools will help them answer questions like: Where are we seeing increases in high-cost medication use?” he continued. “Who is able to keep their CHF patients out of the hospital? How are they doing that? Does one organization do a better job of coordinating services with nursing homes after surgery? If so, what do they do differently?  These insights will help them better align with payers, providers and employers across Michigan as they improve quality and manage cost.”

Healthy Planet is aimed at helping providers receive reimbursement as part of the value-based care system for positive patient health outcomes among populations. The platform includes a suite of reports, dashboards, and clinical workflow tools to enable care management and patient monitoring.

Epic’s population health management platform is one of many modules the health IT company offers to help providers in a variety of specialties improve patient care delivery and meet federal requirements.

Other Epic modules include EpicCare Ambulatory, EpicCare Everywhere, EpicCare Home Health, and others.
